Boy I never realized how tough it is to keep your fingers close to the fretboard...

Pick a scale, some riffs or a lesson you know in your sleep. Only pay attention to how far your fingers are coming off the fret board and ignore everything else for about 15 min a day. It has help me with my flyaway pinky.

Like any other tecnhnuique it must be pratcised........ Try to fucu on keeping the fingers closed and then it after some time it will come naturall for the fingers.......

I think it is quite the oposite thing, for a hammer-on you should hammer the string mainly from the big knuckle of each finger, and at the same time use the weight of the hand to your advantage. The catual movement is like closing your fist. Being close is nice, but you must first be aware of the minimun distance you need to gat a note to ring cleanly.
The other factor is to press just next to the fret not on the fret, and using the very tip of the finger (each finger press with different parts of the tip) so all the finger joints are flexed and the weight is transmited.
