Guitar Setup, Andrew C, side question for you. Options

I've had my guitar since 94' and besides changing the strings and having a humbucker upgraded, I haven't done much with it. It's a nice Ibanez, with a non locking tremolo. It stays in tune pretty nice, but I feel there's excess noise with distortion. I have three pickups, one being a single coil and same results in each position.

So the questions I have are:

How often should you have your guitar adjusted/setup?

Will a proper adjusted guitar get rid of those little buzzing and such?

Last a question for Andrew C, since I know you have a POD XTL also, when using the Veriac Plexi (6D) factory setting (I'm just using this as an example, it's not the only one), do you get background buzzing noise when playing? I understand the muting concept and keeping things from sounding out, but even when doing that I get that noise. Thats through headphones, and amp.
