Over the years I had this problem many times too. And always it was my own fault. I read the tips from Todd and he is so right!! Although I know it's wrong I very often use too much pressure not only killing my fingers but also, sometimes , playing out of tune! And that's nasty. Thanks to GMC I'm aware of this and constantly I'm trying to avoid.
Another problem is my own enthousiasm, motivation and drive. Basically that's a gift but sometimes my mind wants more than my body can handle. Like Todd said : if you feel pain: Stop! Pain can never be a goal in guitarplaying. If your hands are still very sore from the previous "workout", don't go into another 2 hour mindblowing speedplaying, ignoring pain! Have a day rest instead or play relaxed some licks . Next day, recuperation will hopefully done it's job and you will be stronger and faster than before.
It's like sport, training and especially the way you train is crucial, but so is resting and recuperation.
