I recently discovered an useful exercise on the Jeff Loomis instructional DVD where the main pattern is like this
--------------------5---6-5-7-6-8-7- etc ---5---6-5-7-6-8-7----8------------- -----8------------------------------ You can invert it and back to lower strings with a similar pattern. I like to wash the hands in warm water for a while, and then warm up by playing slow scales, modal approach. Then it's off to exercise!

I used to do these hand stretchings and massages, and later play a few slower licks. Nowadays I warm up my fretting hand by playing some Bach pieces, it get's me through

I don't really warm up. I do stretches sometimes. When I feel that it's too cold here and I can't play normally, I just put my hands into warm water, it really helps. My warmup is pretty regimented. Here's what works for me. 1.)A few minutes of wrist rolls, some hand/finger stretches, and range of motion work. 2.)Start slow and work up some speed with some simple drills like this. 3.)Push to max speed and start working on the days material/practice/writing/etc 4.)Cool down by playing some chords 5.)Ice down my forearms with an ice cube 6.)Cool down stretches and wrist rolls. I gave myself carpal tunnel several years back so I'm very careful to warmup and cool down.
