I had (and still have) a lot of struggles with the double stops, especially the ringfinger double stop slide. It seems I can't figure out a way to reliably mute the G string while still having enough pressure on the D string for a consistent tone. Hi man, this is not bad at all.. it just needs a bit of work.
Firs of all, you said you have a real difficulty with the middle finger slide. You do a good job of this, despite it. If you watch Jerry in the lesson, you'll see he also uses his 2nd finger to enforce the 3rd finger. This adds strength and rigidity to the 3rd finger, which will help you with this manoeuvre. For the rest of it, your timing is good but you just need to spend more time with these chord changes so they become smoother. I also don't know if anybody has mentioned this before but the way you hold the pick may be holding you back. If you have it between the thumb and back of the index finger like you do then it is more likely to be affected by coming into contact with the strings, because the finger and thumb is more likely to flex when it meets pressure. If you have the pick between the side of the index finger and thumb then there is more rigidity there when it meets the string. I think that you've received the right feedback here. That 3rd finger slide is the issues that don't let this one sound consistent. Ben's suggestion is the way to go in order to give more strength to this finger, and it's also the trick used for vibrato and bending. The overall lesson needs more practice in order to be able to play everything more relaxed and with more confidence so the best would be to isolate the slide to be worked as an exercise and keep this one in your diary routine to be played at least 3 times per day while you work on other stuff.
