Do you feel relaxed when just jamming around(playing simple stuff) and not practicing?
Cause if you do then you just need more skills and you'll be more relaxed even when playing/practicing harder stuff. Remember,no one is 100% relaxed when playing cause muscles are in use,a lot. -------------------- Youtube

Usually when you play something that you are not comfortable with, your hand starts to tense up. Also after a long session, everybody's hands will be tired. Muscles just get tired.
The most effective way to stay relaxed as possible is to - practice relaxed. This way your brain will actually remember to play everything you practiced relaxed, so later when you do play, your hands will not be stiffen. This helps a lot, and building relaxed technique is something that takes a while, so be sure to practice everything nice and slowly, and let your arms create a flow while playing. -------------------- - Ivan's Video Chat Lesson Notes HERE

You got some great advices up there...I would add my experience, I tend to pick/hit harder with my right hand when I'm on a gig or practice session in the studio..Adrenaline takes its tool and I'm all wired up so I hit harder..Once you catch yourself being less relaxed try relaxing your picking hand by lowering strength you are putting into picking etc and you will definitly see a big change..Once I catch myself picking too hard on gigs etc I just do that and I can really feel the difference...Otherwise I would get tired much faster and that will show on my performance.. In the end its all in your mind to control it (of course as you develop your playing skills you will be more relaxed when playing).

I second that , definitly send a video of you playing (when you don't feel relaxed) , it probably contains some clues why is it happening

One thing to look at is where the weight of both arms goes resting, this weight disposition should change durng playing cause you dont stay quiet.
